Is It Normal to Still Feel Swollen After Surgery?

Swelling after surgery is expected—but how much is too much? And how long is it supposed to last? 

If you’ve recently undergone a joint replacement, spinal procedure, or soft-tissue surgery and you’re still seeing puffiness in your leg, ankle, or incision site weeks later, you’re not alone. In the first 1 to 2 weeks post-op, this response is expected. It is your body doing the work of healing. But when swelling persists for 3, 4, or even 6 weeks, it can become dysfunctional

When Is Swelling No Longer Normal? 

Although timelines vary depending on the type of surgery, here are some general guidelines for when swelling needs professional attention: 

  • Swelling increases instead of decreases after the second week 
  • The area feels warm, hard, or painful to the touch 
  • Skin becomes shiny or discolored 
  • Swelling worsens after exercise or daily activity 
  • You have difficulty putting on shoes, pants, or walking comfortably 

These signs could indicate chronic inflammation, poor lymphatic drainage, or limited circulation — all of which are treatable with physical therapy. 

What Prolonged Swelling Can Do to Your Recovery 

Swelling that doesn’t resolve can affect your recovery in many ways: 

  • Delays healing by preventing full blood and oxygen circulation 
  • Inhibits movement due to joint stiffness or pressure

  • Avoiding long periods of sitting or standing still 
  • Using cold therapy carefully (only under your therapist’s guidance) 
  • Monitoring your symptoms and reporting changes
